Modern Patient Record Challenges
Electronic progress notes used to be a nightmare to read, especially in the Emergency Department. They were reaching 5, 7, even 15 pages long, full of regulatory language and audit trail signatures and time stamps.
These long notes were particularly hard to read on a screen, making it a tedious task to find the crucial elements. You'd have to scroll through page after page to find the Assessment/Plan (A/P) section.

In an EHR, it's not uncommon to have to scroll 12 pages down to find the A/P, only to find it's not even on the last page, but rather on page 10. This was a major problem in the Emergency Department, where doctors would often give up looking for more thinking from other doctors.
The situation was so bad that doctors would stop looking for more information after just a few notes, simply because it was too hard to read. This made it difficult to provide comprehensive patient care.
The adoption of APSO notes in the EHR was a game-changer, making progress notes much more readable and concise. In fact, APSO notes were so much better that it was worth mandating them from the ground up, especially during a major disruption like installing a new EHR.
